Class Application form




    Terms and Conditions:


    • All class fees should be paid in advance before student joins in the new course.
    • In the event of withdrawal from a course student must provide notice of such intention in writing or by email to info@cosmic-dance.com.
    • A 10% late charge will be applied if fees have not been paid within 2 weeks of starting a new package.
    • Students will receive at least 24 hour notice and 100% refund if cancellation occurs due to activities of Cosmic Dance conflicting with the time of the class and a substitute class will be provided.
